The following two files in Adobe PDF format are available for you to download, print, and fill out ahead of time before your appointment if you wish to decrease time spent in office. (Most people spend up to 10 minutes thoughtfully entering the information.)
- Patient Intake Form (PDF file, 72 KB) Demographic information, general health questionnaire, and personal and family eye health survey.
- Privacy Practices Acknowledgment Form (PDF file, 82 KB) Contains an abbreviated privacy policy, for full privacy practices information click here.
If the patient is a minor, the parent or guardian must sign both of these forms.
