The following file in Adobe PDF format is available for you to download, print, and fill out ahead of time before your appointment if you wish to decrease time spent in office. (Some people spend up to 10 minutes thoughtfully entering the information.)

If the patient is a minor, the parent or guardian must sign the forms.
